Head Protection and Concussion in Ice Skating Survey

We are seeking participants to provide information on head protection and concussion in ice skating. We are particularly interested in learning how well alternatives to helmets such as headbands/halos perform during head impacts. Please answer questions as honestly as possible. If you have received more than one concussion, please provide details of the most recent (you may submit additional surveys for previous concussions if you wish).
